Registration Information
  • Conference registration includes all-day access to presentations and vendor area, along with a complimentary lunch.
  • To pre-register by mail, this form must be postmarked by or on February 25, 2007. No exceptions.
  • Any registration form postmarked after February 25, 2007, is subject to on-site fees and will be processed on-site.
  • Online registration will be available until February 25, 2007.
  • Credit cards are accepted through PayPal if you register online. No faxed registration forms will be accepted.
  • Payment must accompany registration forms. Registrations received without payments will not be processed. Please refer to conference fee schedule to avoid additional charges.
  • Group rates apply to 5 or more persons. Group rates are not available after February 25, 2007. Purchase order or personal checks must be included in the same envelope as the registration forms.
  • Requests for refunds (less $10 processing fee per person) must be made in writing to the address below before February 25, 2007. No refunds after February 25, 2007.
  • To confirm receipt of registration forms and payment, contact Jelen Rodecker, Registration Chair, by e-mail at [email protected] after February 25, 2007. Receipt of registration forms will not be confirmed by phone or fax.
  • Conference packets will be given out on-site on March 10, 2007, starting at 7:30 am.
